Accessing Forms

Many forms are available via paper, email, or via our housing software. To request assistance with or accesses to a form, please contact us at uas.housing@alaska.edu.

Housing Applications

Housing Application and Agreement to live on campus for single (non-family) students. Prior to accessing an application, new students will be asked to complete a Housing Eligibility Form. These forms are made available on the same dates as Housing Applications and are accessed through Student Housing Self Service.

Housing Application and Agreement to live on campus for students who are married and/or have dependent children. New students will be asked to provide more information regarding eligibility in the application. This application is made available through the Housing Applications and is accessed through Student Housing Self Service.

Housing Supplemental Forms

Students who elect to live in Gender Inclusive Housing (GIH) must complete a supplemental agreement agreeing to the terms of GIH. This is included in the Housing Application. For more information about Gender Inclusive Housing, visit the Accommodations page.

Students who have an approved Service or Assistance animal must complete an Animal Agreement prior to the animal arriving on campus. For more information about approved animals, visit the Accommodations page.

Access:  Student Housing Self-Service

Form Availability: This will be made available to students upon approval. 

Use this form to register a vehicle with UAS Housing. Following submission of this form, a parking permit will be placed in your mailbox.

Access:  Student Housing Self-Service

Use this form to register a bicycle or other recreational vehicle (such as a Kayak) with UAS Housing. Following submission of this form, a recreation permit will be placed in your mailbox.

This form is used to request a move-in prior to the published move-in day for any semester. Students will be expected to upload documentation of a need to move in early, including proof of early coursework, employment, travel restrictions, etc.

This form is used to request a move-out after the published move-out day for any semester. Students will be expected to upload documentation of a need to move out late, including proof of late coursework, employment, travel restrictions, etc.

Used to file an exemption for one or more housing immunization requirements. 

Access: Request by emailing uas.housing@alaska.edu

Used to register a weapon for storage or set up an appointment for check-out of a weapon with Residence Life. 

Access: Request by emailing uas.housing@alaska.edu

Used to register a guest for an extended stay (more than 1 night), and no more than 10 nights in any one semester.
