You must be currently registered to use this form. Use this form to add, drop, withdraw, change to credit/audit for individual courses.
Change of Major
Only for undergraduate and MPA students currently admitted to a program. If adding a higher degree (certificate, associate, or bachelor) with the occupational endorsement certificate as your current program, you must apply for admission.
Change of Name, Address, or Phone
To request a change to name, marital status, and/or Social Security number for an employee, student, and/or vendor. Employees and students can change their address online using UA Online.

Eaglecrest Enrollment Verification
Education Record Information Release-FERPA
Allows release of all or only a portion of your education record to a particular entity for one academic year. One form must be completed for each recipient.
Send official certification of enrollment for a particular semester. If more than three (3) semesters need to be certified, please request a copy of your official transcripts instead. Enrollment verification is also available through the National Student Clearinghouse at UAOnline.
Release of Directory Information
Directory information includes name, e-mail, home city and state, dates of attendance at UAS, program/major field(s) of study, degrees and certificates received, including dates, participation in officially recognized university activities, Chancellor’s List and Dean’s List recognition, and academic and co-curricular honors, awards, and scholarships, including dates received.

Room Reservation Request
When space is available, many UAS facilities are rentable for community and local events, including: professional development meetings, conferences, screenings, and performances that support the UAS mission, and enrich the vibrancy of the arts, culture, and education in Southeast Alaska.
As per university regulation, the student registration process requires students younger than 18, including those who have graduated from high school or earned a GED, and their parent/guardians to sign this authorization for university enrollment.

Application for Graduation & Commencement
Application for Resident Tuition Assessment
Complete this form to apply for resident tuition assessment and if you plan to enroll in more than 4 credits a semester. Submit it along with supporting documentation to the appropriate campus office prior to the published deadline to add or drop regular semester-length courses in the semester for which residency is sought.
